Continuing Care Pilot Project at Cassidy Lake
In the fall of 2009, Portage implemented a
Continuing Care pilot project at
Cassidy Lake. This program ensures two years of individualised follow-up for all Atlantic youth who have spent time at Cassidy Lake, regardless of how long they have remained in rehabilitation treatment. The impact has been carefully observed over time and the Continuing Care model has shown good results.
Since the pilot project’s inception, the youth that have participated have been very receptive to the program and pleased that Portage continues to show interest in their well-being even if they did not complete their treatment program. Some have returned to Cassidy Lake and others are staying in touch.
Due to the success of the pilot project, the Continuing Care model has been reproduced in other Portage rehabilitation treatment centres across Canada. The program, which uses Portage’s partnerships with social service networks, ensures that youth remain in a comprehensive system of care.
